2-50 Employees

Groups with 2-50 employees must meet the following reform guidelines to purchase a health plan in New Jersey:

  • A minimum of two eligible employees who work 25 hours per week or more.
  • Employer must contribute a minimum of 10% of the employee monthly premium.
  • 75% of employee participation is required.

If your group meets these, please fill out a census form to obtain group health quotes.

Over 50 Employees

Groups over 50 lives do not follow NJ reform rules. We would need the following information emailed, faxed, or mailed to us to complete your quote request:

  • Full census including zip codes
  • Current and renewal rates (if applicable)
  • Current list bill
  • Number of years with current insurance company
  • Employee contribution (employee/dependent)
  • Claims experience (if applicable)
  • Nature of business
  • Plan design

Group Long Term Disability

If your business meets the following guidelines, please complete a long term disability census form for a quote.

  • Minimum of two employees who work 30 hours or more per week.
  • Group must be in business for at least 2 years.
  • Only 50% of the insured’s may be members of the same family.

Group Long Term Care

Groups need a minimum of two employees who work 30 hours or more per week. It may be necessary to complete a medical application or paramedical examination. Please complete the long term care census for a group quote.

Group Life Insurance

Groups must have a minimum of two employees who work 30 hours or more per week to be eligible for group life insurance. You can obtain a quote based on a flat rate or a multiple of your salary. Please complete the group census form to receive a life insurance quote.

Group Dental Insurance

Groups must have a minimum of two employees who work a minimum of 25 hours per week to be eligible for group dental insurance. Employee participation requirements for group dental are as follows:

  • 2-9 lives - 100% of eligible employees must enroll
  • 10-24 lives - 90% of eligible employees must enroll
  • 25+ lives - 75% of eligible employees must enroll

Note - a minimum of 5 eligible employees must be in the group for orthodontic benefits.

There is a six-month waiting period on major and orthodontic services if there were no prior dental benefits for a minimum of six months. Please complete the group census to obtain group dental quotes.
